Custom Gift Form Instructions
In the first part, fill in your first and last name, the best number to reach you at, your e-mail address, your mailing address with your city and state, and your birthday.
In the second part, fill in the gift recipient's first and last name, their number, the address you want the gift delivered to as well as the city.
In the final part, fill in any known allergies of the recipient and a theme for the gift (could be a special occasion, holiday, color scheme, or something unique such as pet lover or sports team).
If you don't want a theme, enter "none".
You can also enter "none" in any of the following areas.
Enter any and all drinks (coffee, tea, Diet Coke, water, etc.), foods (popcorn, sausage, crackers, jam, etc.), sweets (chocolate chip cookies, biscotti, snack-size chocolates, assorted candy bars, etc.), and any extra items (journal, plush bear, mug, bath items, measuring cups, balloon, photo frame, etc.)
Enter the date in which you would like the gift delivered, a message from you to the recipient, an exact price or price range, and any additional information you would like to include.
